**Please Note: All memberships are on a RECURRING BASIS (monthly or annually), if your payment method declines for the recurring payment to renew your membership, your membership will be effectively terminated and you will not be able to re-sign up or have access to membership pricing in the future. Recurring memberships can be cancelled IN WRITING 30 days before the recurring payment is due. It is the patient's sole responsibility to update our office on new payment information to keep membership active****
